Patient Information

On your first visit to Pediatric ENT Associates in Birmingham, AL with your child, you will be asked to fill out some paperwork for us. This paperwork can be filled out at the clinic, or if you’d like, you can download the forms to print and fill out at home to save time.

Pre-Registration

We are committed to making your hospital experience as smooth and efficient as possible. At Pediatric ENT Associates, we understand that your time is valuable, and in an effort to expedite your appointment we are offering an online pre-registration process. 24 hours before your child’s scheduled appointment, you can go online and complete the pre-registration process. This will significantly reduce the amount of time needed for your child’s appointment, as filling out the pre-registration information can be a lengthy process at the clinic.

Pediatric ENT Associates has clinic locations at two Children’s of Alabama campuses. Please be sure to clarify beforehand the location of your appointment.

If your child is scheduled for surgery, the surgery may be done at Children’s South or the Benjamin Russel Hospital for Children. If the surgery is scheduled at Children’s South, see the above directions.
